Assessment just means assess (test) what they know. It doesn't even have to be a formal test. Teachers test informally all the time through observation. However in this case I am assuming they want a formal test. Typically, especially in Math, this is a paper pencil test. However there are many projects you can do to assess students as well.

Yes, you most definitely can. I've physical pains among other scary things that can happen in real life, but not in a dream. Neveretheless, still I felt them. Dreams can be as visible and clear if not more than real life.
